Lost, Stolen, or Destroyed Check
If a paycheck has been lost or stolen, please notify the Payroll Department immediately. The Payroll Department will arrange a stop payment on the original check. A replacement, by check or direct deposit, will be issued as soon as verification has been received from the bank that the original check has not been cashed. The employee will be required to sign a Check Replacement Request Form [pdf] stating that the original check has not been negotiated and if found, it will be returned to the Payroll Department.
If a paycheck has been destroyed or is void due to the issue date, please return the check to the Payroll Department. A replacement, by check or direct deposit, will be issued within four business days.
A $35 check replacement fee will be charged to replace a stolen, damaged, lost, or stale dated check.
