Welcome to St. John Fisher College! The information and links below will be helpful for new employees. Your Human Resources Department representative will be glad to answer any questions you may have regarding these documents.

Payroll and Employment Forms To Be Completed

You will complete Section 1 of this document, including signature and date. Please review the valid forms of identification listed on the back and bring the appropriate original documents with you to your meeting.

Direct Deposit [pdf]

St. John Fisher College encourages payroll direct deposit. Please provide all requested information.

Group Life Enrollment Form [pdf]

Your Human Resources representative will assist you in completing the top part of this form. You will need to name beneficiaries for your group life insurance policy.

Web Benefit Enrollment Information [pdf]

Enrollment for the various benefit plans (health insurance, dental insurance, life insurance, spouse/dependent life insurance, flexible spending accounts for health care and dependent care, long term disability, and personal accidental death and dismemberment insurance) takes place via the web at www.eElect.com.

Costs for the various benefits, plus your flexible spending dollars provided by the College, are shown when you log onto the automated enrollment system. More information on these benefits also is available in the St. John Fisher College Employee Handbook [pdf].

You have 30 days from your first day of employment to complete your online enrollment, but you should make your selections as soon as possible in order to insure there is no delay with the start of your benefits.

Retirement Option Form [pdf]

All new employees must complete this form to indicate whether or not they wish to join a St. John Fisher Retirement Plan.

Salary Reduction Agreement [pdf]

To be completed if you elect to enroll in a St. John Fisher College retirement plan. Enrollment and beneficiary forms for First Mercantile are enclosed. If you select TIAA-CREF as your St. John Fisher College retirement plan, please contact the Human Resources Department for the appropriate enrollment forms.

The Summary Plan Description generally describes the benefits to which you may be entitled under the Plan, your rights and your responsibilities. The specific benefits offered under the Plan are: Health Benefits, Dental Benefits, Flexible Benefits, Long Term Disability Benefits, Group Life Insurance Benefits, Voluntary Personal Accident Benefits, Employee Assistance Plan Benefits, and Pre-Tax Premium Benefits. If you would like paper copies of any of these documents, please contact the Human Resources Department.
