Your Human Resources representative will assist you in completing the top part of this form. You will need to name beneficiaries for your group life insurance policy. |

Enrollment for the various benefit plans (health insurance, dental insurance, life insurance, spouse/dependent life insurance, flexible spending accounts for health care and dependent care, long term disability, and personal accidental death and dismemberment insurance) takes place via the web at www.eElect.com. Costs for the various benefits, plus your flexible spending dollars provided by the College, are shown when you log onto the automated enrollment system. More information on these benefits also is available in the St. John Fisher College Employee Handbook [pdf]. You have 30 days from your first day of employment to complete your online enrollment, but you should make your selections as soon as possible in order to insure there is no delay with the start of your benefits. |
